The TSH test and when you need it
Reviewed by Dr. Sanaa Ghoneim, Medical laboratory specialist 21 October 2026
A TSH test measures thyroid-stimulating hormone, and it is often the first test a doctor orders to check how your thyroid is working. You may need it if you have symptoms that could be linked to the thyroid, if you take thyroid treatment that needs follow-up, or as part of a routine checkup when your doctor thinks it is appropriate. It usually needs no fasting and uses an ordinary blood sample.
What is TSH?
The thyroid is a small gland in the neck. Its hormones play a role in the body’s pace, energy and temperature. It works under the direction of the pituitary gland in the brain, which releases a hormone called TSH.
When the pituitary senses that thyroid hormone levels are off, it changes how much TSH it releases. That is why measuring TSH gives an important signal about the thyroid. Doctors often request the thyroid hormones themselves alongside it, such as free T4 and sometimes T3, to complete the picture.
When your doctor may order it
- Symptoms that may come from the thyroid, such as ongoing tiredness, unexplained weight gain or loss, a pounding heartbeat, feeling unusually cold or hot, hair loss, or changes in periods.
- Follow-up of thyroid treatment, so your doctor can adjust the dose.
- Before or during pregnancy, if your doctor considers it appropriate.
- A family history of thyroid problems, or as part of a routine checkup.
These symptoms can have many causes other than the thyroid, which is exactly why your doctor orders the test: to check rather than guess.
How to prepare
- Fasting: usually not needed, unless other tests ordered with it need fasting.
- Biotin: if you take supplements that contain biotin, common in hair, skin and nail products, tell us. Biotin can affect the results of some thyroid tests.
- Thyroid medicine: do not stop it or change its timing on your own. Some doctors prefer the sample before that day’s dose, so ask yours.
- Other medicines: tell us about any medicines or supplements you take.
We confirm the preparation with you on WhatsApp before your appointment.
Home blood draw
We confirm the tests, fasting and timing with you on WhatsApp before the visit, and a technician takes the sample at your home. Your results reach you on WhatsApp.
About your results
A TSH result is understood together with your symptoms, other tests and any treatment you take. Discuss it with the doctor who ordered the test, who will decide whether further tests or a change in treatment are needed.
